Outpatient Day Treatment Services

Specialized outpatient interdisciplinary services for individuals living at home and needing a structured day program to address mild to moderate functional deficits following acquired brain injuries.

Services are designed to provide therapeutic opportunities for acquisition and enhancement of higher-level functional skills in self-care, communication, neuropsychological performance, physical performance, adaptive behavior, socialization, and physical and vocational performance. Treatment is provided under the framework of a full day of structured therapy activities in both the therapy center and the community.

Treatment Setting: Comprehensive treatment setting providing for individual and group therapy located at UTHCT in Tyler, Texas. The Tyler community provides for numerous opportunities for generalization of functional skills gained in therapy settings.

Key Components:

Case Management/Outcome Measurement - Each client is assigned a Case Manager who implements the development and coordination of a treatment plan; coordinates discharge planning; monitors clinical outcomes; and acts as a liaison to family, insurance case managers, and other funding representatives. Our program utilizes the Clinical Outcome System (CLOUT®) a clinical outcome measurement system specifically designed to measure therapeutic progress in post-acute brain injury rehabilitation.

Interdisciplinary Therapy - Clinicians from a broad range of disciplines provide services in a coordinated manner to ensure outcome-focused, functional treatment goals. Clinicians design a treatment plan that addresses functional skills in behavioral, psychosocial, communication, neuropsychological, and physical areas.

Vocational Assessment/Intervention - Individualized vocational services are provided to clients seeking to achieve their identified vocational outcomes. Services are designed to assist the client in the development of realistic employment goals; establishment of a treatment plan to achieve vocational outcomes; and, identification of vocational opportunities and resources in their community.

Behavior Analysis - Behavior analysis approaches allow the treatment team to work with individuals experiencing behavior problems that interfere with treatment and maximize skills acquisition.

Family Involvement - Families are encouraged to participate in educational sessions and be an active member of the treatment team throughout the course of treatment. Involvement in discharge planning is encouraged to maximize successful outcomes.

Individualized Treatment

Upon admission, each client receives a comprehensive assessment identifying functional deficits and abilities. Specific therapeutic goals and a treatment plan are then developed to determine the sequence for addressing specified skill development.

Appropriate Referrals

Individuals ages 18 and older with a diagnosis of acquired brain injury exhibiting medical stability.

Individuals who are exhibiting mild behaviors interfering with ability to functional independently at home, work, and community.

Individuals requiring a structured therapy program necessary to address functional deficits.
