Outcomes Measurement System
NeuroRestorative® Specialty Services @ UTHCT continually measures the effectiveness and efficiency of its clinical services by examining clinical outcomes of all persons served. Our Clinical Outcome System (CLOUT®) is designed to assess and measure functional abilities critical to the individual’s ability to perform daily living activities independently. Clinicians use the CLOUT® System to establish rehabilitation goals and to measure progress throughout the continuum of care. Upon discharge, follow-up reviews are conducted to evaluate maintenance of discharge objectives. Aggregate data sets are analyzed to identify trends and/or areas for program evaluation and programmatic adjustments and development.
CLOUT® system ratings (Clinical Outcomes) allow for baseline measures, measures of improvement and evaluation of how accurately treatment teams are able to project outcomes in various functional areas. Attention is given to functional gains made during lengths of stay which were as long as projected, versus lengths of stay which were less than projected. Whereas, all aggregate data is compiled and reviewed every six months for trend analysis, individual client data is collected throughout the rehabilitation stay and used to develop goals, evaluate progress, and identify the need to modify treatment.
Durability of outcomes is assessed by post-discharge follow-up at scheduled intervals. This follow-up identifies whether or not a client has been able to maintain discharge objectives across time. These follow-ups are conducted via telephone at post-discharge intervals including two weeks, three months, six months, and one year. Data is compiled into aggregate format and reviewed every six months as part of the program evaluation.
Although outcome is certainly specific to each individual served, historical data sets indicate the following:
- Persons receiving specialized brain injury rehabilitation following acquired brain injuries can achieve functional gains and thus achieving higher levels of independence;
- Our clinical treatment teams are consistently accurate in terms of projecting outcomes and the duration of treatment necessary to attain functional outcomes;
- Family/Social Support System plays a crucial role in the rehabilitation process.
