Referral

To Make a Referral

  • We accept referrals for medically-stable individuals six years of age or older.
  • We use the Rancho Los Amigos scale to assist in determining appropriate referrals. (Please refer to scale below.)

Referral Process:

  1. Please call our referral coordinator at (800) 743-6802 or click here to email a referral coordinator.
  2. Immediately upon receipt of your referral we will contact you to gather important information about the patient; schedule a clinical evaluation; and arrange to meet with family and/or others involved with the patient. In most cases, the clinical evaluation can be scheduled within 48 hours after the referral is made.
  3. Upon clinical acceptance, and following clinical evaluation, a proposed plan of care is provided to all authorized parties.
  4. Our referral coordinator will work at securing funding qualification, arranging a family tour, and communicating with all parties.
  5. Upon final funding approval our referral coordinator will assist with arrangements to transfer and admit the patient.

The Rancho Los Amigos scale is an important guideline in determining appropriate referrals. Individuals who are medically stable and are a Rancho IV or higher would be considered for admission.

Level I - No Response: Total Assistance

Level II - Generalized Response: Total Assistance

Level III - Localized Response: Total Assistance

Level IV - Confused/Agitated: Maximal Assistance

Level V - Confused, Inappropriate Non-Agitated: Maximal Assistance

Level VI - Confused, Appropriate: Moderate Assistance

Level VII - Automatic, Appropriate: Minimal Assistance for Daily Living Skills

Level VIII - Purposeful, Appropriate: Stand-By Assistance

Level IX - Purposeful, Appropriate: Stand-By Assistance on Request

Level X - Purposeful, Appropriate: Modified Independent
