Susie Sheridan, M.S., CCC-SLP
Susie Sheridan, M.S., CCC-SLP, Chief Executive Officer, is a Tulsa native who earned her Bachelor of Arts and Sciences degree in Communications in 1984 and a Master of Sciences degree in Communication Disorders-Speech Language Pathology in 1986 from the University of Oklahoma. She previously held the position of Program Representative/Clinical Evaluator at Oklahoma NeuroRestorative Center in Broken Arrow, specializing in education and referral assessment of post acute brain injury individuals for physicians, healthcare professionals, community members and prospective clients and their families. She has been in the allied healthcare environment for 20 years serving as a clinician, manager, director, consultant, marketer, and most recently, small business owner.
Susie has always been involved in the community and helping to promote the industry in which she has been involved. She is a member of the Case Management Society of America and has participated in the Brain Injury Association of Oklahoma, Hospital focus groups, EOMGMA, Good Samaritan Fundraiser Gala Co-Chair, and Pug Rescue of Tulsa County.
Medical Director
Perri Craven, M.D.
Dr. Craven earned her medical degree from the University of Oklahoma College of Medicine and completed her residency requirements in Physical Medicine and Rehabilitation at the University of Kansas Medical Center. Dr. Craven is a board-certified PM&R physician. Currently Medical Director for Oklahoma NeuroSpecialty, she has served as Assistant Professor in the Department of Rehabilitation Medicine at the State University of New York at Buffalo and Physiatrist for the Head Trauma Rehabilitation Unit at Our Lady of Victory Hospital, also in Buffalo.
Director of Nursing
Gina Sides, RN, MBA
Gina Sides, RN, MBA, serves as Director of Nursing for Oklahoma NeuroSpecialty. She received her Associate Degree in Nursing at Tulsa Community College and her Bachelor of Science Degree and Master of Business Administration Degree both from Southern Nazarene University, Tulsa campus. For the past 13 years, she has worked in both the acute hospital setting as well as in long-term care, with her main focus of interest in physical rehabilitation and Alzheimer’s disease. Gina has participated in the Association of Rehab Nursing, the Association for Professional’s in Infection Control & Epidemiology, and Alzheimer’s Research, as well as hospital focus groups. She is certified in ACLS, BLS, Sedation Analgesia, and Nonviolent Crisis Intervention.
Program Case Manager
Beverly Crouch, LMSW
Beveryl Crouch, LMSW, serves as a Program Case Manager. She received her Bachelor's of Social Work from the University of Texas El Paso in 2005 and her Masters of Social Work from New Mexico State University in 2007. Beverly is licensed to practice in the state of Oklahoma. She has worked in a mental health setting as well as senior home care.

Jennie L. Roland, MSW
Jennie L. Roland, MSW, serves as a Program Case Manager for Oklahoma NeuroSpecialty. She completed both
her B.A. and Masters of Social Work degree at the University of Oklahoma in Norman. Jennie provided
diagnostic and clinical services at Kaiser Department of Psychiatry in San Jose, California.
Jennie has also worked as a clinician and group leader for partial hospitalization and day
treatment programs for mental health clients, as well as crisis intervention settings. Jennie
has spent the past 11 years also working in medical social work, providing services in all areas
of hospital settings, home health and hospice.
Behavior Analyst
Najla Kutait-Faulkner, M.ED., LBP, BCBA
Najla Kutait-Faulkner, M.ED., LBP, BCBA, is a Board Certified Behavior Analyst and a Licensed Behavioral Practitioner with over 23 years of experience in the field of behavioral therapy. She graduated from the University of Arkansas-Fayetteville with a B.A. in Psychology and an M.Ed. in Agency Counseling. Najla is a past Chair of the Oklahoma State Advisory Board for Licensed Behavioral Practitioners and is a BCBA charter certificant.
Lead Clinician and Speech Language Pathologist
Kay Headrick, M.S., CCC-SLP
Kay Headrick M.S., CCC-SLP, serves as Lead Clinician and Speech Language Pathologist at Oklahoma NeuroSpecialty. She completed her B.S. in Speech Pathology at Oklahoma State University in 1985 and earned her M.S. in Communications Disorders at the University of Oklahoma Health Sciences Center in 1987. Kay has provided diagnostic and therapeutic services to adolescents and adults with acquired brain injuries in a variety of settings for the past 20 years.

Jennifer Landphair, M.S. CF/SLP
Jennifer Landphair, M.S., CF/SLP, earned her M.S. in Communication Sciences and Disorders/Speech-Language Pathology from the University of Arkansas in Fayetteville, Arkansas, and her B.S. in Communication Sciences and Disorders from Oklahoma State University in Stillwater, Oklahoma. She completed her clinical internship at Oklahoma NeuroSpecialty before joining the staff as a full time Speech Language Pathologist in June 2007.

April Boczar, M.S., CCC-SLP

April Boczar, M.S., CCC-SLP, is Program Representative and Clinical Evaluator for Oklahoma NeuroSpecialty in Tulsa. She received her bachelor’s degree in Communication Sciences and Disorders from the University of Oklahoma and her master’s degree in Speech-Language Pathology from the University of Tulsa. April is a member of the American Speech Language and Hearing Association as well as the Case Management Society of America.
Brad Hendrix
Brad Hendrix graduated from Southeastern Oklahoma State University with a Bachelor of Arts degree. He began his career as a behavioral counselor in a psychiatric/chemical dependency hospital in 1993. Brad is Program Representative and Clinical Evaluator for Oklahoma NeuroSpecialty with his primary areas being Oklahoma City; Wichita, Kansas; and Dallas, Texas. Brad is a member of the Oklahoma Case Management Society of America and is a board member of the Oklahoma Brain Injury Association.
